At RDDHS, We use a HIPPA compliant program to collect and store all personal information. This form will take 4-5 minutes to complete and can be filled out on your smart phone or other device. Having your forms filled out ahead of time cuts down on your wait time in the clinic and helps us care for you more efficiently! This form only needs to be filled out once and will be kept on record for your following appointments.
